We're looking for you if you want to work in municipal primary care with a focus on health and nursing care in ordinary living. We're looking for you as a nurse for the Municipal Primary Care unit to work within the home care team where care support is also included. You'll be part of a team with nurses, but also in an interprofessional team with an occupational therapist, physiotherapist, unit manager, care assistants, and other professionals surrounding the patient with a focus on patient-safe and person-centered care. You'll have great opportunity to plan and organize your workday so it works for you, your patients, and your colleagues. You'll be the responsible nurse for a geographical home care area where daily driving is included. We have varied, stimulating work with an open work environment where we support and help each other with challenging tasks. Main Responsibilities Your main tasks are to coordinate and ensure that specific nursing interventions are planned, implemented, and evaluated, as well as to educate, mentor, and delegate to care assistants. You perform medical assessments, specific nursing interventions, and work in a health-promoting and preventative way. The work involves independently or together with colleagues planning and following up on nursing interventions. Systems supporting us in the work include digital signature lists and the journal system Life Care HSL. Qualifications We're looking for you as a licensed nurse (Swedish license) with commitment and interest in municipal primary care with a focus on home care. You have several years of experience working as a nurse, and it's a merit if you've worked with similar tasks before. We'd like to see specialist training as a district nurse, elderly care, or psychiatry. To succeed in this position, you work independently and take great personal responsibility in your profession and for planning and follow-up of health and nursing care. You value teamwork highly and have a strong interest in working with frail elderly, dementia, and palliative care. As a person, you are stable and flexible, have high sense of responsibility, and the ability to work both independently and in a team. You're driven by challenges and want to develop both within your profession and in municipal primary care. We place great emphasis on personal suitability with the ability to work on different tasks based on the organization's needs. Terms You work days, evenings, and approximately every fifth weekend, with flexible working hours. We can offer you a wellness allowance, individually adapted introduction, mentor and collegial supervision. Valid B driver's license (manual transmission) is required. Two references, at least one supervisor, are provided at interview. In connection with potential employment, we request an extract from the Police's criminal record and suspicion register.
