We are now seeking a colleague to join our palliative advisory team here at Piteå Hospital! What we're looking for We are looking for you who are a licensed nurse, confident in your role and with at least a few years of professional experience. If you have specialist training in palliative care and experience working in specialized palliative care, that is a merit. We place great emphasis on personal qualities. As a person, you are proactive and drive your work forward independently while seeing opportunities for development and improvement. You are flexible and adapt easily to changed conditions in the organization. Since the work involves close collaboration with patients, relatives, and different professional groups, you are relationship-focused and have good collaboration skills. What you will work with Specialized palliative services in the Piteå local healthcare area (Piteå, Älvsbyn, Arvidsjaur, and Arjeplog municipalities) serve patients with palliative care needs, regardless of disease diagnosis, age, or care setting, and consist of a palliative advisory team as well as safety beds for inpatient care on ward 3A at Piteå Hospital. The palliative advisory team within the Piteå local healthcare area consists of two full-time nurses, as well as a doctor and social worker on part-time basis. As a nurse in the palliative advisory team, your main responsibility is to provide advice, consultation, and support to staff caring for patients enrolled in the specialized palliative service in the catchment area. Advice and support are also provided directly to patients and relatives. Questions and consultations regarding non-enrolled patients receiving general palliative care are also handled by the palliative advisory team. Care contacts with the PRT nurse occur via telephone, scheduled and emergency home visits, video technology, visits to the clinic, or ward. The nurse has a coordinating function and serves as a link between primary care, inpatient care, and municipal services. In your work, you will also assist with customized training initiatives, bedside teaching, and mentoring for staff working with palliative care in various settings, as well as participate in coordination meetings with nurses from home care services and municipal care facilities. The work also includes keeping yourself continuously updated on new routines and guidelines for palliative care locally, regionally, nationally, and internationally. You will participate in and contribute to various county and healthcare region networks within specialized palliative care. The advisory team has no direct patient responsibility but is independently responsible for its advice and any interventions. The work therefore requires very good knowledge of palliative care and person-centered working methods. The ability to collaborate within the team and with various partners, as well as responsiveness, is of great importance. A driver's license is a requirement. Two weekend shifts per month may be included on our inpatient ward. Start date by agreement.
