Work Tasks
Working as a nurse with us is varied work where no two days are the same. You conduct assessments and visits to patients' homes, often in a team together with a doctor or another nurse, but independent visits also occur. Collaboration with home care services and other care providers is central to the work. The nurse in the close care team often functions as a coordinator and connecting link to ensure optimal care for the patient. SIP (coordinated individual plan) is created for most patients. Standard nursing tasks occur. If you have experience and feel confident handling medical-technical tasks such as central lines, that is an asset. Tasks such as intravenous infusions and blood transfusions occur in the role.
About the Workplace
Close Care Team Västerås was started in the summer of 2022 with the purpose of complementing the care structures that previously existed.
Close care means that we in the care structure are positioned between primary care/health centers and specialist care at the hospital. Our goal is to serve as the necessary link so that complex and seriously ill patients in Västerås and Surahammar municipalities can have their care needs optimally met at home. We exist for those who have difficulty accessing care, and our mission is to create security at home to prevent patients from being unnecessarily admitted to the hospital.
The team operates from facilities at Ängsgärdet and consists of doctors, nurses, assistant nurses, and a social worker.

Qualifications
We are looking for you who are a licensed nurse with at least five years of experience in the profession. If you are trained as a district nurse and/or have experience caring for patients at home, we consider that an asset. Since you in the role need to be able to communicate in speech and writing with patients, relatives, and colleagues, good knowledge of Swedish is required. A driving license is a requirement for the position.
As a person, you are confident and stable in your profession, empathetic, and have a natural ability to cooperate with others. You are innovative and structured in your work, take responsibility for your tasks, and contribute to a positive work environment. Since we care for patients in their homes, it is also important that you are flexible and can adapt based on the person and situation. We place great emphasis on personal suitability.
Employment Terms
Temporary position for one year, 100% employment according to agreement.
We apply individual and differentiated salary setting.
