Registered Nurse for Home Healthcare – Daytime, Weekdays and Holidays Off

You will work full-time with our home healthcare assignment during daytime hours on weekdays and holidays off. Experience from basic home healthcare, primary care centers, or ASIH is a plus, but what matters most is that you feel confident in your role as a nurse and want to develop home healthcare together with us. About Us Bromma Primary Care Clinic is a personalized primary care center located at Brommaplan with assignments within Region Stockholm. We are a smaller operation with short decision-making paths, high competence, and close collaboration between physicians, nurses, and nursing assistants. We have a growing home healthcare assignment, chronic disease clinics, and active development work where employee ideas are valued. Our home healthcare team currently consists of three nursing assistants, one registered nurse, and one home healthcare physician. One of our valued nurses is moving on to new challenges in the fall, which means we are now seeking their successor. We are therefore looking for you who wants to take on an important role in our home healthcare team and continue developing the service together with us. About the Position As a nurse in home healthcare, you have a key role in daily operations. You work both independently and as part of the team to ensure safe, secure, and person-centered care in patients' homes. You serve as clinical support for our home healthcare nursing assistants and work closely with our district physicians on planning, assessments, and follow-ups. The work is varied and combines patient visits with coordination, planning, and medical assessments. Examples of Responsibilities Plan and coordinate daily home healthcare. Supervise and provide clinical support to home healthcare nursing assistants. Conduct home visits and nursing interventions. Participate in care planning and SIP meetings. Collaborate with physicians, relatives, care coordinators, and other healthcare providers. Manage incoming cases via LifeCare. Respond to our direct phone line for home healthcare patients during part of the workday. Contribute to the development of routines and quality in home healthcare. Who Are You? We are looking for you who are a registered nurse with experience in independent clinical work. It is a plus if you have experience from: basic home healthcare primary care centers ASIH municipality home healthcare As a person, you are organized, confident, and solution-oriented.
