Forellplan consists of two five-story buildings connected by a skyway. There are a total of 124 apartments, of which 60 are dedicated to dementia care and the remaining apartments provide somatic nursing care. 2 position(s). JOB RESPONSIBILITIES Are you a nurse with a big heart, where empathy, care, and compassion are your traits? Then we have the job for you! We are now looking for committed and collaborative nurses for Forellplan Care and Residential Home where you can make a difference every day in our patients' lives. As a nurse at Forellplan Care and Residential Home, you work with standard nursing duties alongside five other nurses and one nursing assistant. You work together as a team with occupational therapists, physiotherapists, and other care personnel. Together you work to promote patient health and well-being. You are responsible for medical assessments, careful documentation, and registration in relevant quality registers. Patient rounds take place at a listed health center with a responsible physician. HSL documentation is done in Treserva, planning is done in TES. Lifecare is used as communication between care providers. Your job duties include: • Being the care coordinator for a number of patients at the home. • Performing medical assessments. • Planning, implementing, and following up on care measures. • Blood tests, infusions, dressing changes, and end-of-life care. • Working in a team with Physiotherapist/physical therapist, occupational therapist, physician. • Guiding and delegating care tasks to colleagues and students. The position is primarily based on daytime work, but the schedule includes evening and weekend shifts. You will then work in our consulting operations that include the municipality's care and residential homes, home healthcare, and service and group homes within LSS and social psychiatry. As a final-year candidate, you must be able to present a criminal record check from the police for work in the home with elderly people and people with disabilities. QUALIFICATIONS You are a skilled nurse who wants to work in a care and residential home providing good care, providing quality of life, security, and warmth to patients who need it most. We are looking for you who: • Are a licensed nurse. • Have experience working in elderly care or similar healthcare. • Have a driver's license (category B) and experience driving a car. • Have good ability to make independent medical assessments. • Are comfortable handling medications. • Are experienced with documentation in digital health record systems, preferably Treserva.
