Hannahemmet care and residential facility is located in Råcksta, Bromma, with proximity to nature and public transportation. The facility has 54 places divided across four dementia care units and two somatic care units. One of the dementia care units has a Finnish orientation, which means there is Finnish-speaking staff and we acknowledge Finnish holidays, traditions, and Finnish culture. A registered nurse is on site from 7 a.m. to 4 p.m. every day of the week. At other times, the care staff contact Klara Kompetens. About the role As a registered nurse with us, you have overall responsibility for care and medical quality. You work in the Welfare journal system (formerly Epsilon) and collaborate closely with physiotherapists, occupational therapists, and other professionals, where you work together using a function-preserving approach – with a focus on strengthening each person's abilities, participation, and quality of life. You are the care coordinator for 18 patients across a total of two units. Your work includes: planning, implementing, and following up on care interventions based on individual needs responsibility for medical quality and ensuring good and safe care guiding and supervising care staff in daily work being the care coordinator for two units collaborating with physicians and other professionals regarding resident care maintaining contact with and supporting family members contributing to the development of work methods and quality in the facility. You work both independently and in close collaboration with other professionals. You participate in the local management group, the facility's improvement council, and attend team meetings on the units. Position: 100%. You work daytime Monday-Friday and every third weekend. Your experience We are looking for someone who: is a licensed registered nurse has at least 2 years of experience has good knowledge of Swedish language in speech and writing has good computer skills. We would appreciate it if you have experience working in elderly care and with people with cognitive impairment.
