You perform medical assessments, plan and coordinate interventions, and collaborate with primary care centers, other healthcare providers, and our call center. The work gives you great opportunity to use your clinical experience and make independent decisions, while you always have the support of an experienced team. You work on a rotating schedule with day, evening, and weekend shifts (2 weekends out of 5). During weekday daytime hours, you work from our office at Sabbatsberg Hospital where you assess incoming referrals, coordinate care services, and ensure that patients receive the right care at the right time. About Us Aleris Basic Home Care Services provides care on behalf of primary care centers in Stockholm County and offers medical services in the patient's home when primary care centers are unable to make home visits. We are part of Aleris Nearby Care, which also includes ASIH, specialized palliative care, Rehab Station, and Uppsala Nearby Acute Care. We are Looking for You Who are a registered nurse Have at least two years of professional experience Have a B driver's license Have experience with TakeCare and good computer skills Speak and write Swedish well It is an advantage if you have worked in home care, primary care, or acute care. We believe you are a person who feels confident in your profession, enjoys working independently, and finds it easy to collaborate with others.
