RESPONSIBILITIES
As a nurse in the on-call team, you work to provide quality primary care to patients aged 18 and older in their home environment. Services are provided in care and housing facilities, ordinary housing, short-term housing, and LSS housing throughout the municipality.
During on-call hours, 2 nurses work in the municipality and you plan and prioritize services based on patients' current needs. The work is independent, varied, and requires you to make safe decisions even in situations that change quickly. Examples of tasks include symptom relief at the end of life, assessments of sudden health deterioration, and other acute or planned healthcare interventions.
You collaborate closely with patients, relatives, care staff, colleagues, and other actors both within the municipality and the region. Together you work to create safe, secure, and person-centered care where the patient's needs are always in focus.
In your work, you use digital tools that support planning, documentation, and communication. You also have support from colleagues and on-call physicians via telephone when the situation requires it.
QUALIFICATIONS
We are looking for you who is a licensed nurse.
If you have work experience from home care, elderly care, or dementia care, we consider that a great advantage. You express yourself easily in speech and writing in Swedish. Good computer skills are required as we work in several different digital systems. It is also an asset if you have experience with the journal systems Viva and Cosmic.
A driver's license is required.
Do you enjoy independent work with good opportunities for collaboration with colleagues in the care team? That's good, because we work with a holistic view of the patient. Your ability to plan, organize, and prioritize work in a responsible manner will come in handy. Our daily work is changeable, which also places demands on commitment, adaptability, and the ability to take advantage of new opportunities.
