With us, you become an important part of an engaged team that works to provide safe, secure, and person-centered care to Gävle's residents. No two days are the same, which makes the work both developmental and stimulating.
Job Duties
In this position, you work within two home healthcare areas in home healthcare North. The assignment does not involve having your own care responsibility for a geographical area. Instead, the focus is on supporting the activities where needs exist and contributing to a flexible and well-functioning care organization. Together with nursing colleagues, assistant nurses, home care staff, and other professionals, you contribute to high-quality care.
The assignment includes:
• Performing planned and unplanned health and medical care services.
• Conducting independent medical assessments.
• Planning, implementing, and following up on health and medical care measures.
• Prioritizing services and making decisions based on the patient's current needs.
• Making assessments during home visits, at care and residential facilities, and via telephone.
• Delegating, mentoring, and supporting care staff.
• Providing advice and support to patients, relatives, and colleagues.
• Documenting according to applicable legislation and guidelines.
• Working in health promotion and prevention with the patient's best interests in focus.
• Collaborating with internal and external stakeholders to create safe and cohesive care.
QUALIFICATIONS
We are looking for someone who:
• Is a licensed nurse.
• Has a B driver's license.
• Has good knowledge of Swedish, both spoken and written.
• Has good computer skills and can document in digital medical record systems.
• Feels confident making medical assessments and working independently.
It is an advantage if you have experience in municipal health and medical care, home healthcare, or elderly care.
