Primary care in Region Jämtland Härjedalen is organizationally part of the Division of Close Care together with the Ambulance Service, Public Dental Care, Psychiatry, and Child and Adolescent Psychiatry. Primary care is organized into 4 close care areas and since January 2022, primary care and ambulance services have been integrated under good and close care. Each close care area is an economic unit led by a manager. Primary care also oversees the region's healthcare advice services, youth clinic, and the close care unit Näva, which are all grant-financed. Bräcke Health Center is a well-functioning health center with a stable permanent staff. We work with a strong team spirit. Our team includes medical secretaries, physicians, district nurses, nursing assistants, midwife, psychologist, social worker, rehabilitation coordinator, and physiotherapists. What distinguishes Bräcke Health Center from city health centers is that we handle everything—patients who would normally have gone to the emergency department if they lived in a city come to us. This makes the work very varied; it's a good mix of traditional health center work and more acute matters. There are good commuting options to Bräcke. 2 position(s). RESPONSIBILITIES Working as a district nurse/nurse at Bräcke Health Center involves traditional health center tasks such as patient consultations, phone advice, and teamwork with physicians and other staff. The work is also very varied and includes acute tasks. As a coordination nurse, you handle our daily communication with the municipality and hospital via LINK, you are responsible for coordinated individual care plans (SIP), and are available on a dedicated phone line for vulnerable patients. You assist the team with physician contacts, conduct home visits for MMSE, SMA, or other interventions. You serve as the hub for expanded primary care at Bräcke Health Center. Initially, the work will consist of 50% as a coordination nurse and 50% at the health center; this may change if the number of patients for the coordination nurse increases. You may base yourself in Bräcke or Kälarne, according to scheduling. Based on your assignment, you have great flexibility in planning your work. QUALIFICATIONS Licensed nurse or further education as a district nurse or in elderly care. You have a genuine interest in people and patient-focused approach. We expect you to contribute your expertise and good patient care together with your colleagues to provide patients with the best possible care. You enjoy your professional role, are engaged, and contribute to a good work environment through initiative and sense of responsibility. Experience with similar tasks is an advantage.
