The Workplace We are now seeking a coordinator for Department 54, located at St Göran Hospital and part of Addiction Center Stockholm's section for acute and 24-hour care. The department is a specialized inpatient care unit consisting of 22 beds where we treat patients according to healthcare regulations HSL, LPT, and LVM, with psychiatric and somatic conditions related to problematic consumption or dependence on alcohol. Job Responsibilities As a coordinator, you will have a stimulating position where significant responsibility rests on you individually, while also requiring collegial collaboration. You play an important role in the care process and planning for each patient. You conduct individual conversations with patients to investigate their social situation and identify their needs for support and interventions. The role also includes ensuring that all team members are updated on the information they need to best assist our patients. You will contact both internal and external stakeholders, such as social workers, municipal case handlers, housing support providers, treatment homes, and others. You participate in network and care plan meetings where an important task is coordinating and collaborating on interventions with municipalities and other partners such as social services and treatment homes. You are the connecting link between different agencies. Qualifications We are looking for someone trained as a nursing assistant with professional credentials, a licensed nurse, or a social worker. Other equivalent education from adult education programs or older nursing assistant training will be individually assessed. You must have good command of the Swedish language. You have good knowledge of the Microsoft Office suite. Meritorious are previous experience with TakeCare and LifeCare systems as well as experience working in addiction care. For patient-facing work within Stockholm County Healthcare Area: - background checks are applied before employment decisions according to SLSO's general instructions for background checks at employment. - employees who will have patient contact must undergo a new employee health examination, according to current directives, which is conducted through occupational health services before employment. - in addition to Swedish professional credentials, good knowledge of the Swedish language corresponding to level C1 according to the Common European Framework of Reference for Languages (CEFR) is required. The same requirements are guidelines for non-credentialed personnel with patient contact.
