Job Description
Your task is to work in a coordinated manner for patients who are on sick leave and whose care needs to be coordinated with other parties such as employers, the Social Insurance Agency, the Employment Service, or municipal social services. In your role as rehabilitation coordinator, your responsibilities include providing support and advice to individual patients and conducting assessments of individuals' rehabilitation needs. You will be responsible for and participate in care plans and meetings with employers. Your tasks include supporting physicians and together developing clear rehabilitation plans. An important part of your role is to coordinate and collaborate both internally and externally, all to support the patient and help prevent and shorten sick leave periods.
Work Team
We are approximately 35 employees at the health center who enjoy working together in an open work environment. As a rehabilitation coordinator with us, you work in close collaboration with the health center's other staff members.
The health center has a physician clinic, specialist nurse clinics, nurse clinic, child health care, psychosocial competence, foot care, and laboratory. We are also responsible for a number of care facilities.
About You
You are trained as a physiotherapist, occupational therapist, nurse, or have other education and experience that we consider equivalent. Experience in primary care and previous work with rehabilitation matters and care coordination is advantageous. We would like to see that you have experience working in a team and that you are familiar with our medical record system Cosmic. You must also speak and write Swedish at C1 level.

You have good teamwork abilities but are also independent in your approach to work. You can make professional assessments, initiate and conclude treatments, and have a good ability to structure your work. You are communicative both verbally and in writing and are a problem-solver both independently and with others. We place great emphasis on personal suitability.
