Job Description
Your task is to work in a coordinated manner for patients who are on sick leave and where care needs to be coordinated with other parties such as employers, the Social Insurance Agency, the Employment Service, or the municipality's social services. In your role as rehabilitation coordinator, your tasks include providing support and advice to individual patients and conducting assessments of individuals' rehabilitation needs. You will be responsible for and participate in care plans and meetings with employers. Your work tasks include supporting physicians and together developing clear rehabilitation plans. An important part of the task is to coordinate and collaborate both internally and externally, all to provide support to the patient and help prevent and shorten sick leave periods.
Work Group
We are approximately 35 employees at the health center who get along well together in an open work environment. As a rehabilitation coordinator with us, you work in close collaboration with the other staff at the health center.
The health center has a doctor's clinic, specialist nurse clinics, nurse clinic, child health care, psychosocial competence, foot care, and laboratory. We also manage a number of care homes.
About You
You are trained as a physical therapist, occupational therapist, nurse, or have other education and experience that we consider equivalent. It is advantageous to have experience in primary care and to have previously worked with rehabilitation issues and care coordination. We would like you to have experience working in a team and to be familiar with our medical record system Cosmic. Furthermore, you need to master Swedish language in speech and writing, corresponding to C1 level.
For you to thrive and fit in with us, your personal qualities are important. We are looking for someone with a high degree of personal maturity. You have good teamwork skills but are also independent in your way of working. You can make professional assessments, initiate and conclude treatments, and have a good ability to structure your work. You are communicative in both speech and writing and are a problem solver both alone and together with others. We place great importance on personal suitability.
