The Psychiatric Outpatient Clinic in Falun is part of specialist psychiatry in Region Dalarna. We offer assessment, treatment, and follow-up for adult patients in need of specialist psychiatric care. We work in teams with the patient at the center and have a clear focus on collaboration, quality, and continuity of care. We are currently engaged in active development work to strengthen our methods and accessibility. Responsibilities As a rehabilitation coordinator, you will play a key role in supporting our patients through their rehabilitation process to facilitate entry or return to working life. Your main responsibilities will include: Supporting patients in their return to work or studies. Internal coordination. For example, participating in team meetings and serving as an advisor and knowledge provider regarding occupational medicine questions. Collaboration with external stakeholders such as the Swedish Social Insurance Agency, the Public Employment Service, employers, and municipal social services. Qualifications Requirements: Licensed physiotherapist, nurse, or occupational therapist, or alternatively another profession deemed suitable by the employer Strong ability to work independently as well as in a team Good knowledge of Swedish, both spoken and written Merits: Computer skills Experience in psychiatry and rehabilitation Experience with the Cosmic patient records system Experience working in Microsoft Office or Microsoft 365 Experience working with various digital meeting services Personal Qualities: You have the ability to build trusting relationships with patients and colleagues. You are a cheerful and positive colleague who enjoys collaborating.
