Process Leader for FACT in Northeastern Skåne in Kristianstad

By implementing integrated working methods, healthcare services and social services can be provided in a more person-centered and cohesive manner. This ensures that people with complex needs receive continuity, comprehensive care, and participation in their care and support. The work is based on the Flexible Assertive Community Treatment (FACT) model, and we are currently in an exploratory phase. JOB RESPONSIBILITIES In your role as process leader, you will lead and coordinate the work to begin implementing the FACT model, in close collaboration with both the municipality and the region. You will be employed by Region Skåne, Adult Psychiatry, but hold a collaboration assignment that means you also represent the six municipalities in northeastern Skåne. Your responsibilities include having a driving and supportive role in exploring and, upon decision, beginning the implementation of integrated working methods through FACT. The assignment involves creating conditions for joint decision-making in a balanced and transparent manner and leading the process forward. Additionally, you will plan for and/or participate in training and provide information about FACT in the subregion. The target group for FACT work includes both children and adults. You will work closely with process leaders who have similar roles for development work in southeastern Skåne, as well as with subregional coordinators for comorbidity and mental health and suicide prevention. There is also a network of FACT process leaders in Region Skåne where knowledge exchange and method development are offered. This position is an initial project employment of approximately 1 year, with the possibility of extension. The primary work location is Kristianstad, but the position involves work throughout northeastern Skåne. QUALIFICATIONS We are looking for someone with an approved healthcare education or other university education that we consider relevant for the position. If you belong to a regulated profession, Swedish language skills corresponding to at least level C1 according to the Council of Europe scale are required. For non-regulated professions, good knowledge of Swedish in speech and writing is required. Experience with collaboration work and project management between region and municipality is meriting. Furthermore, experience with FACT, work in psychiatry, and a class B driver's license are meriting. As a person, you are structured and accustomed to working independently. Additionally, you are flexible and enjoy both collaborating and cooperating with others. You have good communication skills, are attentive, and can meet people in different roles and contexts.
