By implementing integrated working methods, healthcare services and social services can be provided in a more person-centered and cohesive manner. This ensures that people with complex needs receive continuity, comprehensiveness, and participation in their care and support.
The work is based on the Flexible Assertive Community Treatment (FACT) model, and we are currently in an exploratory phase. To advance the development work further, we are now seeking a process leader for FACT.

DUTIES
In your role as process leader, you will lead and coordinate the work of initiating the implementation of the FACT model in close collaboration with both the municipality and the region. Your employment is with Region Skåne, Adult Psychiatry, but you will have a collaboration assignment that means you also represent the six municipalities in northeastern Skåne.
Your responsibilities include taking a driving and supportive role in exploring and, upon decision, initiating the implementation of integrated working methods through FACT. The assignment involves creating conditions for joint decisions in a balanced and transparent manner and leading the process forward. Additionally, you will plan for and/or participate in training and provide information about FACT in the subregion. The target group for FACT work includes both children and adults.
You will work closely with process leaders who have corresponding roles for development work in southeastern Skåne and with the subregional coordinators for comorbidity as well as mental health and suicide prevention. There is also a network of FACT process leaders in Region Skåne where knowledge exchange and method development are offered. The position is an initial project employment of approximately 1 year, with the possibility of extension. The main placement location is Kristianstad, but the position includes work throughout northeastern Skåne.
QUALIFICATIONS
We are looking for someone with approved healthcare education or other university education that we consider relevant for the position. If you belong to a regulated profession, Swedish language skills corresponding to at least level C1 according to the Council of Europe's scale is a requirement. For non-regulated professions, good knowledge of Swedish in speech and writing is required. Experience with collaboration work and project management between region and municipality is advantageous. Furthermore, experience with FACT, work within psychiatry, and a category B driving license are advantageous.
As a person, you are structured and accustomed to working independently. You are also flexible and enjoy both collaborating and cooperating with others. You have good communication skills, are attentive, and can meet people in different roles and contexts. Furthermore, you work from a holistic perspective in your various processes.
