Personal Advocate in northern Dalarna is a collaboration between Mora, Orsa, Älvdalen, Rättvik, Leksand, Vansbro, and Malung-Sälens municipalities.
As a Personal Advocate, you work to support people with mental health challenges in asserting their rights, strengthening their independence, and coordinating contacts with various authorities.
The activities also aim to promote collaboration between authorities based on the individual's needs and create conditions for better access to care, support, and services with the goal of enabling the person to live a more independent life.
You are based in Mora municipality and conduct client, home, and authority visits in Mora and the collaborating municipalities.
Work Tasks
You work on behalf of the client and contribute to increased empowerment, security, and control in everyday life. The work includes assessing and mapping needs together with the client, coordinating and planning interventions based on the individual's situation. You assist the client in contacts with and between various authorities.
The work also includes identifying gaps in the health and care system and contributing to improvements at the local level, for example by promoting collaboration between different actors. It also involves discovering deficiencies in health and care systems that can lead to improvements at the local level, such as improved cooperation between different authorities.
Qualifications
We are looking for someone with relevant post-secondary education, such as a social worker, behavioral scientist, or nurse with a focus on psychiatry, or other education deemed relevant by the employer.
You have good knowledge of the community's support and welfare systems and experience working with people living with mental health challenges. You must also have very strong communication skills in both spoken and written Swedish, enabling you to communicate clearly and professionally in various contexts, as well as experience with digital work methods and the ability to plan, structure, and follow up work toward clear goals.
Requirements
A Class B driver's license is required for employment as the position involves business travel.
