The Health and Care Unit is part of social services activities directed toward care homes, home health services, and functional support. Our multidisciplinary team provides qualified healthcare with the patient in focus. JOB RESPONSIBILITIES As a nurse in our municipal primary care, you will have a varied workday where you work with different areas within the organization. We are in an exciting, challenging, and developmental period with the transition to Nearby Care, which is being implemented nationwide through cooperation between the municipality and region. This means we actively work to drive the development of efficient primary care in rural and sparsely populated areas. For us, collaboration and good communication are central to our development work and something we prioritize highly within the work group. In this position, you are offered the opportunity to work within municipal primary care activities, directed toward home health services. During on-call periods, you will alternate between different areas within municipal primary care. This includes home health services, care homes, and functional support areas. Your tasks include standard nursing duties such as: • Performing medical assessments, planning care interventions, and carrying out treatments • Providing healthcare and support to patients as well as support to their family members • Working preventively and promoting health through development and quality improvement work • Collaborating internally and externally with various actors • Providing support and guidance to care personnel and students • Documenting patient information and follow-ups according to applicable guidelines Some on-call duty may occur by agreement. QUALIFICATIONS • Registered Nurse/Specialist Nurse • Requirement for driver's license (Class B) • Good knowledge of Swedish, both spoken and written • Ability to work independently and in a team • Empathic ability and interest in patient needs • Interest in change and development work Meritorious: Further education as a specialist nurse and experience in home health services or elderly care. ATTRACTIVE WORK HOURS As part of developing our operations and creating good conditions at the workplace, we are testing a new way of working. In this position, you therefore have the opportunity to work fewer hours for a full-time salary. You will receive a project employment that means you work 90% with compensation equivalent to 100% full-time employment during the project period. You will then be offered permanent employment. If the project demonstrates improved work environment and strengthened staffing, the reduced working hours may be made permanent at the workplace.
