Occupational Therapist for Extended Temporary Position in Home Discharge Team in Kävlinge Municipality

We are now seeking you to work in our home discharge team on an extended temporary basis from August 24, 2026 to August 31, 2027, with the possibility of extension. The home discharge team in Kävlinge Municipality consists of two occupational therapists, one physical therapist, specialist nurses, registered nurses, and one HSL nursing assistant who work closely together. Our home discharge team works in close collaboration with SVU case managers to best coordinate safe discharges for our residents in Kävlinge. The position involves working across the entire municipality and enables safe and secure transitions home from inpatient care. Your work includes conducting activity assessments and training, testing and prescribing physical and cognitive aids, and writing certificates for home modifications. You will mentor and train nursing care staff and supervise students. Your collaboration is characterized by an active and responsible role with a focus on excellent service.
