Habilitation services is a specialist service for people of all ages with congenital or early-acquired disabilities. We work both in teams and independently, always with the individual's health and strengths in focus. We are now seeking you to work as a coordinator at adult habilitation services in Ljungby. Adult habilitation services serve individuals throughout Kronoberg County with congenital or early-acquired disabilities. Our patient population includes people with varying degrees of autism, multiple disabilities, intellectual disabilities, and various mobility impairments. We work throughout Kronoberg County. 1 position(s). DUTIES As a coordinator at adult habilitation services, you will provide information about societal support and services, and assess cases. You plan habilitation interventions and offer counseling and support conversations about disabilities, both individually and in groups, to patients and their networks. You also handle LSS matters. In your work, you will collaborate closely with patients' families and other agencies, such as healthcare providers, authorities, schools, and regions. You are responsible for documentation, issuing certificates, and participate in improvement and development work within habilitation services. We work in multidisciplinary teams in close collaboration with individuals and their networks. QUALIFICATIONS You have: • A degree in social work (Socionomexamen) • Driver's license (Category B) • Commitment and willingness to learn habilitation services work. Since the work is largely team-based, it is important that you are relationship-oriented, have good collaboration skills, and the ability to work independently. We seek a structured and goal-oriented person who organizes and plans their work well. You should be able to handle stress constructively and maintain composure under pressure. We value that you are confident in yourself even in uncertain situations, that you stand up for your views, have the courage to speak up when needed, and can separate issues from personalities. Additionally, it is important that you take initiative in social settings, step forward, and find it easy to build new relationships. We consider it meriting if you: • Are a licensed healthcare coordinator (legitimerad hälso- och sjukvårdskurator). • Have experience working with people with disabilities. • Have experience working within LSS legislation or equivalent.
