The Rehabilitation Center is a base unit consisting of child and youth rehabilitation, specialized support and rehabilitation for adults, as well as vision rehabilitation and hearing rehabilitation. The operations provide counseling and other personal support according to LSS as well as rehabilitation and habilitation services to children, youth, and adults with permanent functional limitations. The Rehabilitation Center is a regional operation with approximately 170 employees with workplaces in Lycksele, Skellefteå, and Umeå. The operation's mission is to contribute to good health and the best possible functional ability, as well as good conditions for an independent life and active participation in society for the children, youth, and adults we serve. We are seeking a coordinator for Youth and Adult Rehabilitation in Umeå. We provide rehabilitation services as well as advice and support under the Health and Medical Care Act to youth and adults with significant and/or permanent functional limitations, as well as to individuals with intellectual disabilities and/or autism. We offer flexible working hours and the opportunity for wellness hours during work time or a wellness allowance. 1 position(s). WORK TASKS As a coordinator with us, you contribute knowledge and commitment that help people feel secure. In your role, you provide psychosocial support to youth and adults with functional limitations and to their next of kin. Work tasks include collaboration with the individual's network, such as representatives, staff groups, psychiatry, social services, health and medical care, the Social Insurance Agency, and the Employment Agency. The work often takes place in the individual's immediate environment and through consultative interventions. You participate in planning and conducting training and group activities for clients/next of kin and their network. Beyond your own professional work, you are also responsible for case management and individual planning, as well as student supervision. The work team includes coordinators, psychologists, physiotherapists, speech-language pathologists, and occupational therapists. You contribute your specific expertise to the team. You can expect well-established onboarding and everything is done with the support of competent colleagues with extensive experience. QUALIFICATIONS We are looking for someone with a degree in social work, preferably with further specialization. You also have good knowledge of significant and permanent functional limitations and the consequences they can have on a person's life. You have experience working with people with psychiatric and neuropsychiatric diagnoses. You are able to collaborate effectively and share knowledge with the individual, family, and other networks, such as the Social Insurance Agency, Employment Agency, and social services, regarding the consequences of the functional limitation. Working in a team as well as independently comes naturally to you. It is advantageous if you have experience working with youth and adults with functional limitations and are a licensed health and medical care coordinator. We will place high value on personal qualities and we seek someone who is responsive, respectful of the client/individual's and next of kin's needs, has good planning and organizational ability, strong collaboration skills, and a clear interest in working in interdisciplinary teams. We are looking for someone who, through their personality, is expected to be a complement to others in the work team. You must have a class B driver's license as the work involves travel.
