About Us Habilitation Center Mörby Adults is a specialist clinic that is part of the Rehabilitation & Health operations. We offer services to people over 18 years old with congenital or early-onset functional disabilities that create difficulties in daily life, such as autism spectrum disorder, intellectual disability, or mobility impairment. By providing advice, support, and treatment to patients and their networks, we can prevent and reduce difficulties that a functional disability may cause. You will work in a multidisciplinary team with an occupational therapist, psychologist, case manager, physiotherapist, and speech therapist. Our goal is to create the best possible conditions for a functioning and meaningful everyday life. About the Position We are looking for a case manager (trained social worker) for our unit. As a case manager, you are responsible for the case management perspective in close collaboration with other professions at the unit. Your work includes: Counseling regarding societal support. Counseling support to patients and relatives regarding the consequences of functional disability. Work with patients and their networks regarding problem-causing behaviors. The work is carried out both individually and in a team with other professions at our clinic. We are a work group with a pleasant work environment and good working conditions. Qualifications You have a degree in social work and preferably experience in rehabilitation and work with people in the target group with intellectual disabilities and/or mobility impairments. You are driven by exploring and trying to understand patients who rarely function according to norms, who require significant adaptations in work methods and perspective-taking, and who require creating individual and creative solutions to problems within existing care processes. You enjoy working independently as well as in a team and take personal responsibility for planning, developing, and advancing patient work in a structured manner.
