Job Description The position involves alternating work in the emergency department and a neurological inpatient ward. As a coordinator in the emergency department, you meet patients with varying psychosocial needs, such as unsustainable home situations, worry and anxiety. Your work tasks also include interventions for patients who have suffered severe illness and traumatic events such as accidents, assault, as well as support for relatives in case of death. Acute psychological crisis support, assessment conversations, support with contact with authorities and other societal actors, and information about society's resources are common interventions. You collaborate with the multiprofessional team to provide patients and relatives with the psychosocial support needed in acute situations. Since the activity is characterized by acute interventions, you get varied work with days that are typically not planned in advance and at times a high work pace. On the neurological inpatient ward, people who have acutely fallen ill with stroke and other neurological diagnoses are cared for. Work tasks mainly consist of assessment conversations, crisis management in a changed life situation for both patients and relatives, and support to the patient and loved ones based on psychosocial and social legal factors. Work is conducted in close collaboration with the multiprofessional team. Working hours are daytime, Monday through Friday. Work Group The employment is in the Rehab Group, part of Movement and Health, which is part of Central Care Region Östergötland. The unit provides coordinators, occupational therapists, and physiotherapists within the hospital's operations. The coordinators in the Rehab Group work with various clinics at the university hospital and have continuous professional meetings, joint supervision, and collaborate during absences. About You We are looking for someone with a degree in social work. Experience in coordinator work within health and medical care and/or further education in the field, and if you are a licensed health and medical care coordinator is meriting. You are interested in development and have a professional approach and the ability to organize and prioritize your work. It is important that you have good initiative, are flexible, and are used to working both independently and in teams with other professional categories. You need to have good knowledge of Swedish, both spoken and written, and possess a driver's license. As a person, you are secure and open. Furthermore, you are flexible and enjoy working in an activity with short care periods, varied work tasks, acute matters, and at times a high work pace.
