The Social Services Department's Employment Unit (AME) has as its primary task to support unemployed residents in various ways toward sustainable self-sufficiency. AME works, among other things, on assignment from the Swedish Public Employment Service and the Support and Subsistence Unit within the Social Services Department.

As a case manager, you will be an important part of creating structure, direction, and momentum in participants' processes. You work closely with the individual, but also together with colleagues and collaboration partners to find sustainable paths forward. WORK TASKS
In your role as a case manager, you receive cases, map the individual's conditions, and establish individual plans together with the participant. You follow the individual's process toward work, studies, or other self-sufficiency and ensure that interventions are based on the individual's goals, resources, and needs.
The planning is based on knowledge from research and proven experience regarding what strengthens people's opportunities to approach the labor market. You meet people with various types of challenges, such as mental, physical, and social health issues, harmful substance use and addiction problems, as well as people with limited knowledge of Swedish.
An important part of the work is collaboration. You work with commissioners, employers, healthcare, authorities, and other internal and external actors to create cohesive and effective planning around the individual. Documentation, monitoring, and evaluation are a natural part of the work.
Together with your colleagues, you work to provide each participant with individually tailored and effective support. We work unpretentiously in the team and share responsibility, knowledge, and experience so that the operations as a whole function in the best possible way.
As a case manager, you will also spend a smaller part of your position mentoring participants in our practical activities. You will also plan and conduct lectures, workshops, and courses with the aim of strengthening participants' knowledge, motivation, and opportunities for work or studies.

QUALIFICATIONS
We are looking for someone with a relevant bachelor's degree for the position, such as social worker, behavioral scientist, career counselor, physiotherapist, occupational therapist, or other education that the employer deems equivalent.
You have experience working with employment measures, rehabilitation processes, or corresponding support for people who need extra help to approach work or studies. You have experience meeting people with different types of life challenges, such as physical or mental health issues, social problems, addiction problems, or establishment in a new society.
We see that you are confident in individual conversations and have experience planning and conducting group activities, such as lectures, workshops, or educational initiatives.
Requirements for the position are:
• relevant bachelor's degree,
• experience working with people in change processes,
• good ability to express yourself in Swedish in speech and writing,
• good digital skills and experience with documentation,
• B-class driver's license.
It is meritorious if you have:
• experience with employment-focused rehabilitation,
• experience with IPS, ACT, MI, BIP, KUB, or AWP,
• experience with collaboration between different authorities, healthcare providers, and employers.
Employment form: Permanent employment.
Duration: Indefinite, start date: Start date by agreement.
