Case Manager SoL/LSS

We are seeking someone who wants to contribute to Future Hagfors - together we develop good and close care and the social services of the future. Job Responsibilities Assess needs and make decisions on interventions according to SoL and/or LSS. Follow up on approved interventions. Participate in various collaboration formats for the individual together with relatives, service providers, and other actors. Be a key person in coordinated care planning together with other actors. Play an important role in other collaboration with relatives, representatives, service providers, healthcare, and other authorities. Good balance between work and leisure. Qualifications We are seeking someone who is confident in your professional role and enjoys working in an environment where no two days are alike. You have good analytical skills and can express yourself well in speech and writing, as documentation is a large part of your work tasks. Education: Social work degree, social care program, or other equivalent education. Knowledge: Good knowledge of LSS (Act on Support and Service for Persons with Certain Disabilities), SoL (Social Services Act), and FL (Administrative Procedure Act). Experience: Previous experience in public administration is a merit. Language: Very good Swedish language skills, both spoken and written. Driver's License: Category B driving license required.
