Welcome to us The Health and Care function (HS) in Jönköping Municipality consists of 13 interprofessional HS Teams, as well as a Home Team. We have specialist focus areas in psychiatry, palliative care, home rehabilitation, dementia, and functional support. Together, we are approximately 460 committed employees. HS-Team 12 is part of the Health and Care function serving the areas of Central West, South Jönköping, Torpa, Munksjöstaden, and Fridhem. Within the area there are three elderly care facilities: Bruksgatans äldreboende, Kristinedals äldreboende, and Tornets äldreboende, as well as Brunnsgatan service housing. The work group operates from the team office at Skeppsbron and consists of physical therapists/physiotherapists, occupational therapists, nurses, specialist nurses, nursing assistants, and unit administrator. Your tasks consist of conducting assessments and offering, implementing, and evaluating the health and care interventions required based on the individual's rehabilitation needs. You work with a health-promoting focus on strengthening citizens' own resources and rehabilitation capacity. When needed, you also guide and train family members and home care/nursing personnel. In your role, you have close collaboration with both primary and specialized care to adapt care needs based on the individual. With us, you also work interprofessionally and have close cooperation with other roles such as occupational therapists, nurses, nursing assistants, rehabilitation assistants, and benefits officers. Your expertise You are a licensed physical therapist/physiotherapist We prefer that you have work experience in the field as a physical therapist/physiotherapist, rehabilitation assistant, or other role in health and care Good knowledge of Swedish, both spoken and written, as well as good computer skills are required The work demands good physical mobility as work occurs in home environments where unforeseen situations arise and must be solved in a manner appropriate to the situation. A B-class driver's license and experience driving a car is a requirement As a person, you are self-driven, resourceful, and organized. It is important that you feel confident and take responsibility for moving your processes forward efficiently. At the same time, you are adaptable to changing circumstances and thrive in an activity where days vary. You collaborate well with other people and are clear in your communication style.
