Within Umeå Municipality, you work as a nurse with health and medical care services, handling medications, palliative care, documentation, assessments, and supervising other staff. You are responsible for ensuring that medical interventions function well for our patients and coordinate contact with doctors and other professionals in primary care and hospitals. We are now looking for you to work part-time as a nurse in health and medical home care, care and residential facilities, evening and night shifts, and Umeå Northern Short-Term Center! Health and Medical Home Care (HSIH) Health and Medical Home Care is responsible for home healthcare, rehabilitation, habilitation, and assistive devices in ordinary housing for people over 18 years old. We work from a person-centered approach where each patient encounter is unique and meaningful. You perform health and medical care services at the primary care level. You plan your workday to a great extent, prioritizing between different tasks and taking your own initiatives. You collaborate closely with patients, relatives, colleagues, and other important actors, both within the municipality and the region, to create the best possible conditions for safe and quality care in patients' own homes. Umeå Northern Short-Term Center Umeå Northern Short-Term Center is organizationally part of home healthcare, where the nurse is responsible for medical interventions up to the primary care level. The short-term facility offers care for a limited period for people who, for various reasons, cannot have their needs met at home. This may include rehabilitation after hospital stays, the need for respite for relatives, greater care needs, or time spent waiting for specialized housing. The goal is to strengthen the individual's ability to return to their everyday life at home, with support from home services and home healthcare. Care and Residential Facilities At care and residential facilities within Umeå Municipality, you work as a nurse with responsibility for health and medical care services for our residents. Tasks include medical assessments, wound care, medication management, documentation, and supervision of care staff. You ensure that medical interventions maintain good quality and function well in daily work. In this role, you collaborate closely with care staff, paramedical professionals, and the manager at the care and residential facility, and you coordinate contact with doctors and other professionals in both primary care and hospital care. Evening and Night Shifts Evening and night work suits many – perhaps you who like having your mornings free and want to find time for life outside of work. Perhaps you are currently working part-time or have just retired? Experience from work in health and medical home care, care and residential facilities, and/or elderly care is desirable. You have good initiative and are good at thinking innovatively. You have the ability to work independently while recognizing the importance of collaboration with various professionals, where the individual themselves also plays an active role. Of course, you have a respectful demeanor and are attentive and open in your interactions with both those receiving care and their relatives. You are good at collaborating and want to contribute to good communication and cooperation with other actors. Great emphasis is placed on personal suitability. For you to be able to communicate with users, relatives, and colleagues, write and access care documentation, you must have good knowledge of the Swedish language in both speaking and writing. A valid driver's license is required. Animals may be present in the workplace and some people smoke, so you should not have allergies. Heavy tasks may occur but assistive devices are available.
