City Health Center, Eskilstuna About Us City Health Center is an well-established primary care clinic with over 17,000 registered patients and approximately 70 employees. All professions within primary care are represented at the facility. We also have a child health center and family center. City Health Center is located in the center of Eskilstuna. Primary care on-call services are provided on weekends at City Health Center's facilities in collaboration with other health centers in Eskilstuna. We work with the Close Care transformation, which means that we co-create with residents and shift focus to working more person-centered, cohesive, proactive, and health-promoting. Your Responsibilities The work consists of a constant variety of tasks such as telephone consultations, digital care contacts, and clinic visits. On-call shifts at primary care services occur. You manage your own work but also contribute to the overall operation of the health center and develop the business together with us. Your responsibilities include collaboration with other professions on simple and complex conditions, telephone consultations, clinical assessments and interventions with a person-centered approach. You make independent assessments, conduct examinations, and triage to the appropriate level of care. You meet your patients in person, virtually, or by phone. Your Qualifications We are looking for you who are a licensed district nurse or registered nurse who wants to work at a pleasant workplace characterized by joy at work. Previous work experience and experience from working in primary care are merits but not requirements; we would be pleased if you have prescribing rights.
