Piteå Health Center has approximately 10,100 registered patients and is one of Region Norrbotten's largest health centers. We work in three interprofessional teams where doctors, nurses, nursing assistants, and rehabilitation staff work together to care for their registered patients. We are now looking for District Nurses who want to work broadly within primary care and be part of a team where patients are followed over time and where collaboration between professions is a natural part of daily work.
What we are looking for
You are a licensed district nurse with strong knowledge of Swedish, both spoken and written. It is an advantage if you have experience from primary care, telephone advice, or your own clinic. If you have education or experience in, for example, diabetes, asthma/COPD, or elderly health, that is also valuable. We are looking for someone who can make independent assessments, prioritize between different tasks, and who enjoys collaborating with other professions around patient needs.
What you will work with
As a district nurse, you will work with:
Telephone advice and medical assessments
Clinic visits with your own booked patients
Health promotion and preventive measures
Follow-up of patients with chronic diseases
Coordination for patients with extensive care needs
Collaboration with municipalities, specialist care, and other actors when needed
You will be part of one of our teams and participate in team rounds together with other professions. There, patient cases are planned, prioritized, and followed up jointly. The goal is for patients to meet a team that is familiar with their situation and works in a coordinated manner.

Information about the position
The position is full-time, permanent, working hours 08:00 - 17:00. Start date by agreement. Selection and interviews are ongoing and the position may be filled before the final application deadline.
Region Norrbotten applies individual salary setting according to collective agreements.
