We are now seeking a district nurse for Bräcke Healthcare Center Nyhälsan in Nässjö, with a branch office in Forserum.
Your Role in the Team
As a district nurse with us, you have your own clinic and follow patients through assessment, treatment, and follow-up. An important part of the role is to work preventively and promote health, where continuity in patient contact provides the opportunity to observe changes over time and adapt support according to patient needs. You work independently in your clinic, but with colleagues nearby when you need to discuss or gain additional perspectives. Together with physicians, nurses, care coordinators, rehabilitation staff, and the psychosocial team, you contribute the district nurse perspective in the work around the patient. You are also part of developing the clinic and can contribute with both experience and ideas.
Practical Information About the Position
The work is carried out at both the healthcare center in Nässjö and the branch in Forserum. Some service at the nearby clinic in Eksjö is also part of the assignment. The position is a permanent full-time employment, but an employment level from 80 percent can be discussed.

Nyhälsan is a healthcare center in Nässjö with a branch in Forserum and approximately 7,800 registered patients. We work with the full primary care assignment and have specialized clinics in, among other areas, diabetes and asthma/COPD. Continuity and long-term patient relationships are important to us, with multiple areas of expertise close at hand in the organization.

You are accustomed to taking responsibility for your own patients and making independent assessments. At the same time, you enjoy working closely with other professionals around the patient. Qualifications
Swedish license as a nurse
Advanced education as a district nurse
Experience in primary care and clinic work
Strong knowledge of Swedish, spoken and written
Meritorious
Competence in asthma/COPD or diabetes
Experience with digital care
Other education or experience relevant to the primary care assignment
