Piteå Health Center has approximately 10,100 listed patients and is one of Region Norrbotten's largest health centers. We work in three interprofessional teams where doctors, nurses, nursing assistants, and rehabilitation staff together are responsible for their listed patients.
We are now seeking District Nurses who want to work broadly within primary care and be part of a team where patients are followed over time and where collaboration between professions is a natural part of everyday work.
What we are looking for
You are a licensed district nurse and have good knowledge of Swedish, both spoken and written.
It is advantageous if you have experience from primary care, telephone counseling, or running your own clinic. If you have education or experience in, for example, diabetes, asthma/COPD, or the health of elderly patients, that is also valuable.
We are looking for someone who can make independent assessments, prioritize between different tasks, and who enjoys collaborating with other professional groups around the patient's needs.
What you will work with
As a district nurse, you will work with:
Telephone counseling and medical assessments
Clinic visits with your own scheduled patients
Health promotion and preventive measures
Follow-up of patients with chronic diseases
Coordination for patients with extensive care needs
Collaboration with municipalities, specialist care, and other actors when needed
You will be part of one of our teams and participate in team rounds together with other professions. There, patient cases are planned, prioritized, and followed up collectively. The goal is for patients to meet a team that knows their situation and works in a coordinated manner.
With us, there is an opportunity to develop special areas of responsibility based on the organization's needs and your interests.
What we offer you
Work with great variation between scheduled and acute patient cases
Opportunity to combine independent work with teamwork
Adapted introduction based on previous experience
Regular professional development
Wellness allowance and other benefits within Region Norrbotten
