Vårdcentralen Cityhälsan Centrum is centrally located on the former hospital grounds in Norrköping. Here you will find an operation with approximately 14,100 registered residents in an average population and almost 60 permanent employees. With us, you work in multidisciplinary teams where collaboration and continuity are central. We have four larger teams at the health center and since spring, also smaller teams where each doctor has their own nurse. This creates good conditions for close cooperation, continuity in patient work, and a working method where we utilize each other's expertise. With a stable basic structure in the operation, you also receive good support in your daily work. We also have several specialist departments, including an elderly care clinic, and are an LGBTQI-certified health center. This reflects our ambition to offer inclusive, high-quality care and a workplace where different perspectives and needs are taken seriously. It gives you a varied day, stimulating pace, and close collaboration with many different professions. We are an IBH health center (IBH=integrated behavioral health), which means collaboration between doctors, psychologists, rehabilitation coordinators, and nurses, to the benefit and learning of both patients and staff. The goal is to work together across the entire health center to increase many patients' functional level and quality of life, and to enhance patients' ability to manage their symptoms and avoid medicalization. Please also read about the region's career ladder for nurses and your benefits with us. Job Description We are seeking a diabetes nurse for a permanent position. As a diabetes nurse at the health center, you will have a diabetes clinic together with a colleague. A diabetes-responsible doctor is also part of the team. You will manage diabetes patients at the clinic with relative independence and collaborate with the respective primary care physicians. We strive for continuity in the patient's contact with both doctor and diabetes nurse. The work is a mix of in-person clinic visits, telephone follow-ups, and administrative work. Participation in continuing education is encouraged. As a nurse with us, you also take on the role of resource nurse, where the nurse in collaboration with the resource doctor and resource nursing assistant handles acute situations at the health center. Two mornings a week we have a minor acute care clinic staffed by a doctor and nurse who together care for scheduled patients. It is possible that you will also work in one of these functions, which can contribute to variety and skills development. Telephone advice is the entry point for our patients and accessibility is incredibly important for the entire health center to function. We consider it important that all nurses maintain their competence in telephone advice and that everyone can contribute to this work when needed. You will therefore also work to some extent with telephone advice in an extent that we will agree upon. About You We are looking for someone who is a licensed nurse/district nurse and who has experience in diabetes care. Primary care experience and district nurse training are meriting, as is training in the diabetes field. You are a positive person with great flexibility and who finds it easy to collaborate. You are engaged and interested in primary care. You enjoy being part of a larger context and in your professional role you are empathetic and concerned with maintaining good relationships with others. You are also someone who has good organization and prioritization skills.
