DUTIES As a diabetes nurse at Söderhamn Din hälsocentral, you are responsible for the daily operations of the diabetes clinic. You work independently with patient consultations while being part of a multiprofessional team with close collaboration with physicians, rehabilitation staff, and other professionals to provide patients with safe and high-quality care. You are interested in development and improvement work and confident in your professional role, with the ability to meet people in a professional and empathetic manner. In the role of diabetes nurse, you should • be a licensed nurse or district nurse with specialization in diabetes • hold a valid B-class driver's license. For this position, it is advantageous if you have experience working in primary care, experience with elderly team care, Lifecare, and the Cosmic medical record system.
