Psychosis Care Stockholm is a newly established service within Stockholm County Health Care Region. Here we gather care for patients over 18 years old with psychotic illness in one organizational unit. We offer assessment, investigation, and treatment according to national guidelines and regional care programs. Description Specialist Psychiatric Reception 5, S:t Eriksplan is centrally located in the Atlas area near Vasapark. The reception has excellent public transportation connections with buses, subway, and commuter trains. Our psychosis reception is a unique workplace and we who work here are very proud of the qualitative and often life-saving work we perform. We are committed to offering high accessibility and a well-functioning care chain, where outpatient and round-the-clock care work closely together. We offer assessment and treatment based on regional care programs and national guidelines. The clinic has several ongoing development projects. Specialist Psychiatric Reception 5, S:t Eriksplan is primarily responsible for patients with psychotic illness living in Stockholm's inner city (Kungsholmen, Norrmalm, and Östermalm) but also has some private patients from other areas. At the reception, which has 30 employees, we work in multidisciplinary teams consisting of physicians, nurses, Case Managers, psychologists, social workers, occupational therapist, rehabilitation coordinator, employment specialist, medical secretaries, and unit manager. A mobile emergency team is available to assist patients during on-call hours. As a case manager at our reception, you work as the "spider in the web" conducting psychiatric treatment independently and collaborating internally and externally with various actors. You are responsible as coordinator/primary care contact to ensure your patients receive the right services together with your team. In addition to this, we would like you to participate in the reception's patient education programs such as patient and family education. Qualifications You are trained as a Case Manager. You have previous experience in psychiatric care and we would like you to have experience with our patient group. It is meriting if you have Step 1 training and have worked with relapse prevention or within addiction care as some of our patient group has comorbidity with substance abuse. You enjoy working together with colleagues in a team to achieve the best for the patient. You should be able to work independently and in a structured manner, but also collaborate internally and externally. Good knowledge of the Take Care documentation system. You have an interest in the patient group and are interested in development work. We place great emphasis on personal suitability. A valid driver's license is meriting as we conduct some home visits. Personal Qualities You are passionate about psychosis care and development work, have good collaboration skills, and value teamwork. You are responsible, solution-oriented, and contribute with creativity and humor. Employment Form Permanent, full-time 40 hours per week. Start date by agreement.
