Care services in Falun Municipality is undergoing an exciting transition toward Good and Nearby Care and a new social services act. We are now strengthening our team with a responsible case handler for discharge processes – a key role for you who want to combine operational work with development and collaboration. As a support case handler, you will be part of our planning team and work with discharge planning from inpatient care and short-term housing to ordinary housing. The role combines operational work with overall responsibility for developing procedures, processes, and collaboration together with the unit manager and section manager. Job Responsibilities You will be a central part of our established planning team and have overall responsibility for discharge processes. You will work both closely with the individual and strategically to develop working methods that make a real difference. In this position you will: Manage and coordinate discharge planning in the team Plan discharges from hospital and short-term care Ensure quality and competence in the work Develop and improve working methods and processes Strengthen collaboration with the region, health centers, and other stakeholders About the Assignment: You will contribute to: Enabling more people to return to independent life at home Reducing care periods and preventing readmissions Creating a cohesive, safe, and person-centered care chain Qualifications The role requires: Several years of experience in public administration with focus on discharge processes Experience leading and instructing colleagues in working methods Personal Qualities We are looking for someone who: Is organized, collaborative, and development-driven Has a holistic perspective and focuses on the individual Is calm and confident in your profession We Offer: An important role in a developing operation Opportunity to influence future working methods Engaged colleagues and close collaboration with healthcare Terms of Employment Permanent full-time employment with a start date by agreement.
