About the Operation Primary care's multimodal rehabilitation team for chronic pain operates at three locations in Dalarna - Ludvika, Falun, and Mora. The team in Ludvika is starting fresh and will work with targeted, coordinated interventions based on a clear biopsychosocial approach. The target group consists of patients with chronic pain (at least 3 months duration), where previous primary care interventions have been insufficient and where there is a need for comprehensive team management. Work focus areas: Physical activity, function, and activity capacity Psychological and social consequences of chronic pain Support for return to work, studies, or other meaningful living The operation is based on the national care pathway for chronic pain and is an important part of Region Dalarna's development of more cohesive and person-centered care. We offer: The opportunity to help build an operation from the ground up Meaningful work with a clear patient focus Work in a multiprofessional team with high expertise Participation in developing Region Dalarna's pain care Structures for learning, development, and experience sharing Job Responsibilities As an occupational therapist/rehabilitation coordinator on the team, you have a central role in supporting patients to regain structure, balance, and function in daily life. You work both independently and in close collaboration with the team, where you together plan and carry out rehabilitation. Work areas for occupational therapist: Occupational therapy assessment Activity focus (for example, the ReDO method) Interventions around structure and strategies (for example, the HaKoll method and the 1177 program Organize Your Day) Lifestyle habits (eating habits, sleep habits, physical activity, and more) Daily life ergonomics Hand/orthoses Assistive devices Other job responsibilities: Coordinate and manage care for patients enrolled in the team; you work both independently and with other professions within and outside the team. Conduct rehabilitation meetings, document care plans, and follow them up with the team. Participate in multimodal team interventions together with physiotherapist, psychologist, and physician Collaborate with health centers, specialist care, and other actors as needed Contribute to follow-up, quality work, and registration in quality registers (NRS) Work with digital tools such as video consultations and support via 1177 Work Approach and Organization Work is characterized by: Clear role distribution and coordination within the team Regular team meetings and shared development Person-centered approach Continuity and holistic perspective on the patient You will be part of a regional context where MMR teams collaborate, learn from each other, and jointly develop working methods and quality. Qualifications Requirements: Licensed occupational therapist Meritorious: Experience in primary care Experience working in a team Experience with or interest in working with patients with chronic pain Personal Qualities Great weight is placed on personal suitability.
