We are now looking for a dedicated occupational therapist for an exciting 12-month project and are expanding our case management with an occupational therapist to create legally secure decisions, safe discharges from hospital, and development of discharge-to-home teams. With us, you will have an important role where, through assessments in the home environment, you contribute to ensuring that the right support is provided at the right time. You work closely with case managers, home care services, healthcare professionals, and other professionals to create the best possible conditions for individuals to live an independent life at home. Responsibilities Conduct functional assessments and activity analyses in the home environment. Assess needs for assistive devices, adaptations, and rehabilitation interventions. Contribute to safe hospital discharges. Provide support to case managers through professional assessments and recommendations. Work preventively and rehabilitatively to strengthen individuals' own resources.
