Your Assignment
You will assess individuals' activity capacity, body function, and environmental factors. By implementing measures such as activity training, assistive device prescription, and environmental adaptation, you will support and promote individuals' ability to be active, engaged, and independent. You will also guide and educate care personnel and collaborate with other team members, including through care planning and team meetings, to develop person-centered health plans.
Description of the Workplace
We provide rehabilitation services for short-term care, residential care facilities, and home healthcare in Söderhamns Municipality. The unit has seven physiotherapists, eight occupational therapists, three rehabilitation assistants, six nursing assistants in Safe Discharge and IHR (intensive home rehabilitation), and one unit manager. We are co-located, which gives us opportunities to develop together. We work patient-centered in teams with nurses, care personnel, unit managers, benefits advisors, and physicians when needed.

We Are Looking for You Who
Are a licensed occupational therapist. Experience working in home healthcare is a merit. A valid driver's license is a requirement for this position.
Your role requires high levels of independence and accountability as you are responsible for planning and organizing your own work. You need to take initiative, prioritize, and make decisions within your professional area of responsibility. Since we work in teams, it is important that you have good collaboration skills, are solution-focused, and keep patient engagement in focus. It is also important to have empathy and good conduct in all situations. You need to easily adapt to changing circumstances, see the bigger picture, and consider the broader perspective. As a person, you are positive and motivated to be part of the organization's continued development.
