You become an important part in strengthening public administration and creating efficient and patient-safe processes around discharge and continued support at home. Your responsibilities Your role includes conducting home assessments and participating in hospital discharge processes. The work also includes support with public administration and decision-making processes, as well as documentation in Cosmic LINK and HSV-journal. An important part of the work is collaboration with public administration, short-term services, home health care, and Region Östergötland. The role also includes prescribing personal assistive devices. Who are you? We are looking for a licensed occupational therapist who wants to work in a collaboration-oriented service where the patient's needs are central. You have experience working with home visits and functional assessments. You have good knowledge of personal and work-related assistive devices. You also have experience collaborating with multiple stakeholders. As a person, you are solution-focused and flexible. You are confident in your profession and have a good ability to work independently. You are also communicative and appreciate collaborating with various actors. To succeed in this position, we see that you have good knowledge of Swedish, both spoken and written. You also have a B driver's license for manual transmission and can cycle.
