Occupational Therapist - Home Rehabilitation and Residential Care | Tiohundra

Welcome to us - We are currently seeking an occupational therapist with primary responsibility for Home Rehabilitation and also with some involvement in residential care. You will be employed in Tiohundra's large Rehabilitation Unit – an organization where we value collaboration, development, and job satisfaction. About the position As an occupational therapist, you will meet patients in their homes, whether in ordinary housing or residential care. You will conduct assessments, plan, implement, and follow up on interventions to support patients in their daily lives. You have rehabilitation responsibility and work closely with the multidisciplinary team, which includes nurses and care staff. Together with the physical therapist, you will also guide and support care staff in ergonomics and a rehabilitation-oriented approach. Supervision of students is a natural part of the role and contributes to the development of future occupational therapists. Your work tasks include, among others: - Conduct activity and mobility assessments based on the patient's home environment - Prescribe assistive devices through needs assessment, testing, adaptation, information, training, and follow-up - Implement rehabilitation and function-maintaining training - Provide advice on adaptations and prepare certificates for housing adaptation applications - Work preventively with fall prevention at the patient's home - Collaborate with family members, team colleagues, and external stakeholders - Document in TakeCare and register time in Intraphone - Supervise students About our offer Our unit has several different work areas combined. About you For this position, you need: - Completed and approved occupational therapy education - B driving license - Good knowledge of Swedish, both spoken and written It is advantageous if you also have: - Experience in the field We are looking for someone who is curious, independent, innovative, and a good listener.
