We provide primary care including physician services, respiratory services, home visits, child health clinics, district nurse services, psychosocial team, occupational therapy, and physiotherapy. JOB RESPONSIBILITIES Are you an occupational therapist who wants to work with us to support our patients' opportunities for activity and health? The position is offered at 100 percent. The need at Hedesunda Färnebo Din Health Center is 50 percent, and the remaining 50 percent involves working as part of the team in the primary care collaboration area Gävle. As an occupational therapist with us, you will work with, among other things: • assessment of activity ability in consultation with the patient and rehabilitation needs • assessments, prescription of assistive devices, fitting, follow-up according to the prescription process • memory assessments as a complement to the district nurse's and physician's assessment/investigation • formulate statements in matters concerning home adaptation and applications for personal assistance • monitoring of assistive device costs and rental responsibility • patient education and counseling • individual treatment to a high degree in collaboration with other professionals • assessments and occupational therapy interventions in the patient's home. You are responsible and solution-focused with good collaboration skills, as we have interdisciplinary cooperation. In the role of occupational therapist you should: • be a licensed occupational therapist • hold a valid B driver's license.
