WORK TASKS You will be part of the reception team at the assessment unit for elderly and collaborate with case managers. Your assignment is to conduct activity/functional assessments in connection with new and/or existing home care services. You will also work with prescribing and following up assistive devices for patients in ordinary housing, leading group training, and working proactively with fall prevention initiatives based on your profession. In this assignment, you work independently in your profession, but collaborate with another rehabilitation colleague. The work is based on motivating independence using a person-centered approach. You are employed at the assessment unit, but may also perform tasks at the rehabilitation unit as needed. Work hours are during daytime. QUALIFICATIONS We are looking for you who are a licensed occupational therapist or licensed physical therapist. Good knowledge of the Swedish language and some computer skills are required as we have documentation responsibilities. Furthermore, you have at least one year of experience in your profession, preferably within municipal health and medical care. A B-class driver's license is required. ABOUT THE WORKPLACE The Assessment Unit for Elderly, LSS & Social Psychiatry, which is part of the Authority and Municipal Primary Care section, consists of 10 dedicated case managers, 1 occupational therapist, and 1 physical therapist. Work at the unit is led by area-specific group leaders and a unit manager. The Assessment Unit is part of the Care and Services Administration.
