1 position(s). Your Workplace The Social Services sector is tasked with contributing to good living conditions throughout life for people living in Härryda municipality. The sector is divided into five operational areas: Care and Support, Functional Support, Health and Assistance, Integration and Employment, and Children and Family. The sector also has a shared administrative unit and a development and follow-up unit. The position is located in the Health Promotion Unit, which is organized under the Health and Assistance operational area. The unit's mission is to offer preventive services with the goal of enabling the target group to strengthen and maintain their health and have good quality of life. This is accomplished through non-needs-tested services such as open senior meetings, group activities, advisory services, and group support. The mandate covers all parts of the municipality. The unit collaborates with various actors both internally and externally, such as associations, societies, and study associations. Within the unit, nine employees work with assignments as leisure assistants, family support consultants, coordinators, health mentors, project managers, and unit manager. A large number of volunteers support the unit's activities with volunteer assignments. We are now expanding our family support services with a new position as a family support consultant with dementia expertise to assist family members who support a loved one with cognitive decline. You will be based centrally in Mölnlycke and work closely with our two family support consultants, Sara and Marie. Your Assignment We are looking for someone who has a genuine interest in working with support for people in vulnerable life situations and who is engaged with issues related to family support and support for people with cognitive decline. In your role, you will be instrumental in developing the new assignment as a family support consultant and dementia coordinator in the social services, as well as developing working methods and collaboration within the dementia field, both internally and externally. Your task is to offer support and guidance to both the family member and the loved one with cognitive decline. You are a support contact and provide advice based on the social services mandate, without patient or medical responsibility. Visits and conversations can take place in the family member's or loved one's home. Support is provided through individual conversations, group conversations, and by providing information about services available to the target group. You coordinate and implement knowledge-building services and health promotion activities such as lectures and training aimed at family members and loved ones. You collaborate with other professionals based on the family member's or loved one's support needs, such as regional primary care, social workers, and other units within social services such as care and support. As family support consultants in the unit, you work together to develop and coordinate family support services in the municipality and to disseminate information about family support. Collaboration is an important part of the work with both internal and external activities, as well as with volunteers and interest organizations. Requirements for the Position · You are a licensed nurse with experience working in health and medical care for the elderly and within the dementia field · You have documented education in the field of dementia diseases · You have knowledge and experience working with people with dementia and in interacting with people with cognitive decline · You have experience with conversation methods and good pedagogical abilities · You have the ability to provide advice and guidance · Good ability to express yourself orally and in writing · B-class driver's license is required. It is advantageous if you have: · Specialist training in dementia or elder care · Knowledge of the social services mandate and working methods · Knowledge or experience in elderly care · Experience supporting family members or collaborating with social services To obtain employment, a certificate from the Police's criminal record register must be provided. A certificate from the suspicion register may also be relevant based on the position.
