The shift toward care closer to home represents a changed approach. We work at our clinic and in patients' homes, which gives us deep knowledge of our patients. We strive for continuity and strong relationships. Based on patient needs, we are ready to implement available technology that can improve and simplify our work processes. Approximately 14,000 patients are registered at the health center, with a significant portion being elderly, which in turn means growing home care services (HSV), currently approximately 160 enrolled patients. At the physician level, we are responsible for three special residential facilities (SÄBO) including short-term beds, totaling approximately 116 places, as well as specialized palliative home care (SSIH in Trosa and Gnesta) with approximately 20 enrolled patients.
Your Responsibilities
In collaboration with the municipality's nurses, you will care for patients in HSV and SÄBO.
In your daily work at the clinic, you will conduct patient visits ranging from scheduled follow-ups for chronic diseases to assessing and managing acute conditions. You will be an important team player working closely with other professionals. You will need to be available by phone to support nurses in HSV and SÄBO. You will also participate in the health center's dementia team in connection with dementia assessments and collaborate with our heart failure nurses. As part of your role, you will mentor ST physicians during their placements with us.
Your Qualifications
We are primarily seeking a licensed physician and specialist in general medicine.
However, we also welcome licensed specialist physicians in geriatrics, palliative care, or internal medicine with strong interest in primary care. Good Swedish language skills and good IT skills are required.
