The Dementia Team is a specialized care and support service that works with people who have or are suspected of having a dementia disease. The team operates centrally from Kållekärr and works closely with the Trygg Hemma team, home care, licensed professionals, benefits administrators, and other partner organizations. Our mission is to contribute to safety, structure, and quality of life for individuals through person-centered and preventive interventions in ordinary housing. We work both practically and in an advisory capacity, and we offer support to family members and training to staff. 1 position(s). WORK TASKS As a specialist nurse in the Dementia Team, you will: • Conduct assessments and observations in the individual's home environment. • Support people with dementia in daily activities. • Work to create structure, safety, and functioning routines. • Provide support, advice, and guidance to family members. • Train and develop the competence of colleagues in care and support. • Participate in collaboration meetings and SIP work. • Contribute to the development of person-centered approaches within the service. • Work in a consultative manner with regular operations to find sustainable solutions based on the individual's needs.
