We are now seeking nursing assistants for the Assessment Team who thrive with varied and independent work and want to work preventatively and rehabilitatively with the individual's own abilities in focus. YOUR ROLE As a nursing assistant on the Assessment Team, you work to map out an individual's possibilities and conditions for managing their daily life at home. This may involve people coming home after a hospital stay or short-term stay, but also people whose health condition or needs have changed at home. You work preventatively and rehabilitatively with a focus on making use of what the individual can do themselves. By observing, supporting, and having dialogue with the individual and their loved ones, you create a picture of what resources and needs exist for the person to live as safely and independently as possible. Documentation is an important part of your work. You document continuously and compile your observations to support the social worker's continued assessment. During the assessment, you have frequent contact with home care, health and medical personnel, and social workers. Your work helps create good conditions for the activities that take over once the assessment is complete. Your work also includes supporting and collaborating with other related activities, mainly within preventive work and public administration. YOUR COMPETENCE We are looking for you who are a trained nursing assistant with experience in care work. To succeed in this role, you need to be able to meet people with different conditions and health statuses and make use of the individual's own resources. You take responsibility, make your own decisions, and take initiative based on the situation and needs. Documentation and written communication are important parts of the work. Therefore, you need to have good knowledge of Swedish, both spoken and written, and feel comfortable working in digital systems. Experience from work where written communication has been a major part of the assignment is meriting, as is experience with a rehabilitative approach or mapping of an individual's support needs in daily life. You thrive in varied work, can adapt when circumstances change, and work well both independently and together with others. You meet the individual and their loved ones in a responsive and accommodating manner and collaborate well with other professions. A B driver's license is a requirement as the work involves travel throughout Skellefteå municipality. Skellefteå municipality is part of the Finnish and Sami administrative area. Language skills in Finnish and Sami as well as cultural competence in these areas are therefore meriting. The position is subject to a requirement for a criminal record certificate according to law. Before a possible employment offer, you must provide an excerpt from the criminal record for work with elderly people or people with disabilities. You are responsible for ordering the correct excerpt from the Police Authority. YOUR NEW WORKPLACE The Assessment Team is part of the Preventive Work and Public Administration business area within care and services. Our mission is to create conditions for a safe and secure return home through early interventions and to map out an individual's possibilities for remaining in their own home. Through observation, dialogue, and documentation, we contribute to creating a clear picture of the individual's needs and possibilities. The approach is based on seeing the whole person and making use of the person's own abilities and conditions. The goal is to clarify what support is needed for daily life to function as independently and safely as possible. The Assessment Team operates from centrally located premises in Skellefteå, but assignments are located throughout the municipality. Here you will become part of a team where collaboration between different professions is an important prerequisite for creating good transitions and the right support for the individual.
