In Hemse, there are four home care teams that work closely together and jointly manage home care services for all of southern Gotland. Your responsibilities In your work, you will, among other things: Provide support, assistance, and motivation based on each client's individual needs and wishes. Perform personal care. Carry out medical interventions on delegation. Assist with meal preparation, accompaniment, shopping, cleaning, and laundry. Work according to clear implementation plans with the individual at the center. Work flexibly according to operational needs, including in other teams or departments. As a contact person, you are also responsible for: Establishing and following up on implementation plans. Documenting interventions in our digital systems. Maintaining contact with relatives. Collaborating with home healthcare, rehabilitation, social benefits administrators, and other home care teams. Using our digital planning tool as support in your daily work. The work is scheduled and includes day shifts, evening shifts, and alternating weekends. For the nursing assistant position in home care, the starting salary range is 29,000–34,000 kronor per month. For the residential care assistant position in home care, the starting salary range is 24,000–30,000 kronor per month. We apply individual salary setting. Final salary is determined based on education, relevant experience, competence, and responsibility in the role, as well as in accordance with applicable collective agreements. Who are you? We are looking for someone who is trained as a nursing assistant and has the right to use the protected professional title according to applicable regulations. For the residential care assistant position, no formal education is required. Experience in healthcare and care is a merit. Five positions are located in our car-based team in Stånga, where a Class B driver's license is required. Two positions are located in our walking and cycling team, where you need to be able to cycle. A Class B driver's license is a merit. You work independently in the client's home while having good collaboration skills and enjoying working with colleagues and other professional groups. You are empathetic and create security through respectful and professional conduct. You have a good overall perspective and see both the client's needs and the organization's goals. With your problem-solving and analytical skills, you can analyze situations, make well-founded decisions, and find solutions to everyday challenges. In accordance with the new language requirement, you need to have a good command of Swedish in both speech and writing to ensure safe care, good and correct documentation.
