The Close Care Team in Eskilstuna works with person-centered care and support for elderly patients with multiple medical conditions living in ordinary housing. The operation is part of the development toward future close care and is built on close collaboration between nursing assistants, nurses, rehabilitation staff and benefit case managers. DUTIES As a nursing assistant at the Close Care Team, you work in the client's home and are an important part of the multidisciplinary team. The role involves significant personal responsibility while also maintaining close collaboration with colleagues from different professions. You are responsible for planning, implementing and following up interventions based on the client's needs and circumstances. Your duties include, among other things: • Care work in accordance with the Social Services Act (SoL). • Delegated health and medical care services. • Assignment as a fixed care contact. • Documentation in accordance with current guidelines. • Collaboration with clients, relatives and other professionals. The work is based on a person-centered approach where the client's needs, resources and participation are in focus. You contribute to creating security and continuity while showing respect for the client's and relatives' integrity. Working hours are scheduled for day, evening and weekends according to the schedule. Driving is included in the position. QUALIFICATIONS We are looking for you who: • Are a trained nursing assistant. • Have proof of protected professional title from the Swedish National Board of Health and Welfare or are covered by transition rules. • Have at least two years of current experience as a nursing assistant. • Have experience working with the elderly. • Have good knowledge of Swedish, both spoken and written. • Are accustomed to documenting and using digital tools via computer and phone. • Have a class B driver's license. It is meritorious if you have experience with: • Working with elderly patients with multiple medical conditions. • Palliative care. • People with cognitive impairment. • Home care or similar activities.
