The Social Services Department's mission is to meet people's needs for security. DUTIES At Solbackens Short-Term Residential Care, you will work as a Primary Contact Person and be responsible, together with a number of care assistants in a contact person group, for actively driving work toward the goals that apply to the users the group is responsible for. Short-term care involves variation and places high demands on competence as our users come and go during shorter periods for rehabilitation and recovery after accidents or illness. In short-term care, you work in a cheerful and committed team with good collaboration and close support from colleagues, nurses, rehabilitation staff and the unit manager. Currently, active development work is underway focusing on professionalizing the work by developing the contact person role, where nursing assistants take greater responsibility for following up on users' goals and processes. As an employee, you are expected to have an interest in and be involved in this work. You will also be involved in starting up short-term care at the new care and residential facility Rallarrosen with expected occupancy in May 2027. You will work with: • Ensuring a good reception upon admission and ensuring that important needs, risks and goals are quickly identified and documented. • Establishing, following up and updating implementation plans. • Providing care and support based on the user's needs, goals and implementation plan. • Continuously documenting relevant observations, changes and actions. • Identifying and reporting deviations and risks in operations. • Collaborating with benefits officers, nurses, physiotherapists and occupational therapists. • Being driving force at weekly meetings and multidisciplinary follow-ups so that work meets users' needs and contributes to their goals being achieved. • Contributing to a good discharge and ensuring that important information is handed over when the user moves on to ordinary or special housing. QUALIFICATIONS We are looking for you who: • Are a trained nursing assistant • Have very good skills in Swedish, both spoken and written. • Enjoy taking on greater responsibility and are attracted to challenges. • Have the ability to work independently while also being able to take on a coordinating and driving responsibility for the users you are a contact person for. • Have an interest in quality and want to contribute to quality development in the workplace and continuously develop your own competence. • See good collaboration and well-being at the workplace as an important resource in work with users. • Prioritize friendly and factual communication over personal prestige and enjoy working in a team with plenty of laughter and humor. We consider it a merit if you have at least three years of experience in care and social services or health and medical care. We place great emphasis on personal suitability.
