Activity Description Home Care consists of seven units: Norra, Södra, Nära, Östra, Support Unit, Night Unit, and Resource Unit. All units, except the rural units, are co-located at Upplagsvägen together with home health care nurses, occupational therapists, and physiotherapists. Through individually assessed interventions, we strengthen each person's ability to manage their daily life and enable them to remain in their home in a safe and familiar environment. Work Responsibilities In your role as a home care assistant with planning responsibility, you will be responsible for planning approved services for users in ordinary housing. You will ensure and coordinate daily planning for the team and ensure that the user's needs and wishes are met in a cost-effective manner. You will support the activity in long-term service planning, ensure follow-up, improve co-planning, and be able to manage different tasks in parallel. Document and report according to current procedures. As a planning responsible home care assistant, you will have an outward-facing role, which means you work daily in close cooperation with relatives, nurses, administrators, managers, and staff members in the activities. You will be involved in driving, monitoring, and ensuring quality of service planning. The position includes performing care and support services according to individual implementation plans 50% of the time, both planned and during work peaks. We work based on quality and safety for our users. The users' needs and approved services should form the basis for service planning. Qualifications We are looking for someone with home care assistant education or other training that the employer considers equivalent. It is advantageous if you have experience in home care services or similar activities. You are a pedagogical and communicative person with good ability to express yourself in Swedish both orally and in writing. You have good computer skills, are responsible, thorough, and flexible, and have good ability to work both independently and in a team. You have the ability to manage different tasks in parallel and to plan, organize, and prioritize in work situations where quick decisions must be made. You are service-oriented with the ability to meet both users and employees based on their different roles. Great emphasis is placed on personal suitability. It is advantageous if you have previously worked with planning services for users within health and care. In the activity, we use, among other things, Pulsen Combine, TES, Skyresponse, DF respons. Within the framework of the national minority language law, Gällivare Municipality is the administrative municipality for Sámi, Meänkieli, and Finnish. Knowledge of any of these languages is advantageous when applying for employment in Gällivare Municipality. A Category B driver's license is required. Working Hours/Duration Employment rate 100%.
